Once discharged, your GP can issue a Sick Note, based on your Hospital discharge letter.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Self-Certificate<\/h2>\n\n\n\n<p>If you have been sick for more than four days in a row, but less than seven, you can self\u2013certify your illness using a SC2 form. You can obtain this form from your employer or the <a href=\"https:\/\/www.gov.uk\/government\/publications\/statutory-sick-pay-employees-statement-of-sickness-sc2\">HMRC website<\/a>. <\/p>\n\n\n\n<div class=\"wp-block-file\"><a id=\"wp-block-file--media-0be31583-94f2-4f67-8cc0-b8e4f0b1299f\" href=\"https:\/\/www.harleygrove.nhs.uk\/wp-content\/uploads\/2020\/03\/self_certification_certificate_1-1.pdf\">Self-certification certificate<\/a><a href=\"https:\/\/www.harleygrove.nhs.uk\/wp-content\/uploads\/2020\/03\/self_certification_certificate_1-1.pdf\" class=\"wp-block-file__button wp-element-button\" download aria-describedby=\"wp-block-file--media-0be31583-94f2-4f67-8cc0-b8e4f0b1299f\">Download<\/a><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">More than seven days off sick<\/h2>\n\n\n\n<p>If you&#8217;re off work sick for more than seven days your employer will normally ask for a Fit Note (or Statement of Fitness for Work) from your GP or Hospital Doctor.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How to count&nbsp;sick days<\/h2>\n\n\n\n<p>When you&nbsp;work out the number of days that you&#8217;ve been sick, you need to count all the days in a row you&#8217;ve been sick, including days you don&#8217;t normally work such as weekends and bank holidays.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Statement of Fitness for Work<\/h2>\n\n\n\n<p>The Doctor will provide you with a Statement of Fitness for Work (\u2018Fit Note\u2019) if you are still not well enough to work. <\/p>\n\n\n\n<p>Further information is available at the <a style=\"color: #428bca;\" href=\"http:\/\/www.direct.gov.uk\/en\/MoneyTaxAndBenefits\/BenefitsTaxCreditsAndOtherSupport\/Illorinjured\/DG_175850\">Direct.Gov<\/a> website.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">When do I need a Fit Note?<\/h2>\n\n\n\n<p>The NHS Information website provides further details about Sick or Fit Notes.<\/p>\n\n\n\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link wp-element-button\" href=\"https:\/\/www.nhs.uk\/common-health-questions\/caring-carers-and-long-term-conditions\/when-do-i-need-a-fit-note\/\"> Fit Notes &#8211; What I need to know<\/a><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Private Certificates<\/h2>\n\n\n\n<p>If you or your employer require further information concerning your period(s) of illness , then your Doctor may require to issue you with a letter.
